Our office is now paperless. All patients must fill out and update their online patient profile before their upcoming appointment. Make sure to update your contact, insurance and medical information

  • Please be sure to fill in ALL applicable fields.
  • Finally, click on the Submit Data button at the end to ensure everything gets sent back to us.

  • In preparation, we ask that you read this entire message and follow all directions.

  • If you need to reschedule, please notify us at least 24 hours before your appointment, otherwise you will be charged a cancellation fee of $45.
  • If you wear glasses, please be sure to bring all the glasses you currently use to your appointment.
  • If you are a contact lens wearer, please come in wearing your current contacts. Be sure to know the brand, prescription powers for each eye or bring the box that they came in.
  • Please bring your medical insurance cards and driver's license. We will be scanning these into your records. We should already have your vision insurance information on file.
  • The doctors are requiring Optomap Retinal Imaging to be performed during your annual eye exam. This procedure will reduce or eliminate the need for a dilated eye exam. The fee for this procedure is $39.
  • We request that you park in our assigned parking in the back of our office.
