Please complete your registration form before your first visit. This helps us prepare for your appointment and reduces the amount of paperwork needed when you arrive.
Before you begin
- The form usually takes about 10 minutes.
- Please have your vision and medical insurance cards available.
- Please have your medication list ready.
- If you wear contact lenses, please have your current contact lens boxes or prescription nearby.
If the form does not submit properly, please print or save a copy of your answers and bring it to your appointment.
