Reminder to bring a list of all prescription medications, picture ID, and insurance card(s) to your visit. If you are a contact lens wearer, please wear your contacts to the exam. All Co-Pays are due at the time of the visit.
If you are unable to keep your appointment, we kindly ask that you give a 48-hour notice. No show fees may be charged for late canceled or missed appointments.
