Please take a few moments to complete our online welcome form.

Fill out the tabs in their entirety in order to allow us to provide you with the best possible service at the time of your visit. Please bring your photo ID as well as vision and medical insurance ID cards with you to your appointment.

Please click here and complete the form before your first office visit. This will save you time and make the office administration process easier.

Bring the following items to your exam:

• All eyeglasses and contact lenses (and the contact lens packaging with prescription information) you routinely use, including reading glasses.
• A list of any medications and nutritional supplements you take.
• A list of questions to ask the doctor, especially if you are interested in contact lenses or laser vision correction surgery.
• If you work on a computer, bring the measurement of the distance from your eyes to the screen.
• If you have a hobby such as sewing or piano that requires a specific focus distance, bring the measurement of that distance.
